Decipher fragments were digital objects spread throughout the memories within The Lost Archive.

Trivia

Proximity to one was signalled by 'shimmering' sound effect.
Collecting the fragments constructed an image in the database.
Collecting all of the fragments (20) made it possible to break the Loop.

The "Find all pieces" achievement was awarded for collecting all decipher fragments.
