The only way Ezio's dying moments could be recorded from his perspective is if he learns how to create seals like Altairs over the course of the game, and creates one as he is bleeding out or something. We don't know what they are or how they are created, it could be possible with any random object he has on him at the time. Note: This memory would still end either the moment of his death or the moment he finished the seal, whatever comes first.
