I have theories. Perhaps when Altair wrote in codex about him trying to find some way to extend his life with the apple, the apple didn't make him immortal or extend his life. Perhaps he really died when Mongol attacked, in another way the apple didn't show any effect....or so he thought. Maybe, rather than extended Altair's life it regenerated or reincarnated his life. So, Altair will be reborn as different person, different personality, in different time, from different background but same destiny and also same fate as the apple wants. To help TWCB as their "prophet".

My second theory is more than Adam's descendants, they're also his reincarnation...
