The most recent article said Juno may have capitalized on Desmond's feelings for Lucy and forced him to stab her so he could awaken the 6th sense inside his mind while in the Black Room, so there is a chance she is still alive. Plus, you have to be touching the Apple to be healed--she wasn't touching it. Obviously, Juno saw that Desmond needed help to gain that extra sense, so she sped it up by forcing him into a coma. If Lucy happened to die, then Desmond will have to grieve when he wakes up. Plus, lead characters get killed off all the time in games, so that has nothing to do with it. Remember Lavitz from Legend of Dragoon? He was awesome and Dart's best friend, but he still died.
