the thing is too that the guy had half of a glasgow smile, which means that the side of his face was slit with a knife, ezio's came from a rock, it's not just some kind of random scar, that's the kind of scar someone gets in a serious fight, or mugging. not to mention ezio's glare at him after he grabs his hand, and the knowing look that he gives back, and the templar emblem, the weird thing is that he almost looks somewhat asian like shao jun (though with the animation is is hard to tell ethnicity, he could have just been another italian) maybe the box he gave to shao had something to do with his end, we already know that there have been a ton of templar quasi-assassins during ezio's time, and in response to all the people talking about a poisoning not being dignified enough for him, I don't think it's much more dignified than heart/lung problems from his smoke/carcinogen inhalation from the villa attack/ the ship burnings on his attempt to leave istanbul, not to mention his close proximity/handling of datura powder/smoke during his time in istanbul. He had plenty of reasons that he "could" have died, but the scene and mood are really set to imply a poisoning for whatever reasons from the young man, and while I do not think that he is a hallucination on ezio's part, he did seem to be antagonizing ezio somewhat with his (possibly an act) arrogant attitude, and lecherous/womanizing attitude. I wanna see where ubisoft goes with this.
