Ezio's health was getting worst, probably due to the gunshot in Villa or when he escaped form cappadocia or whatever it's called. The bullet (assuming it is a full on pure lead bullet unlike modern days.) left and infection of he cold have breathed in enough smoke that wold cause very damaging side effects over the years. Ezio wasn't poisoned by Jun or f***face in the pic above. Also remember the night before? He almost got crushed between a tree and a giant man. That would cause rib fractures. Ezio died of Cardiac arrest. The man really isn't anyone. Just some guy who happens to be wearing a bracer seeing Ezio.
