If Desmond were in a dream when he stabbed Lucy, he could have just woken up from it to stop from seeing it, or the voices in the credits would have awakened him. I don't think a dream of stabbing Lucy would be so traumatic that he'd go into shock. The Apple doesn't give visions of the future unless the character is one of TWCB. Minerva was the only one who showed they had this power because she was a superhuman, otherwise Ezio, Desmond and Altair could see what would happen in the future. Because they are flawed by human DNA, maybe they can't see the future through the Apple? If he were in a dream, then them going into the Vault, his visions of Ezio as an old man, escaping to Monteriggioni, all of that did not happen. The dream would take him 2 giant steps back because none of it really happened. Plus, if that Inception concept were true, if he did go into shock, wouldn't that result in limbo because he thought the stabbing was reality? Game over right there. Think of the last scene in Inception.
