1) The specifics of the bans are not final, and subject to change. They could be site-wide, though it's more likely to be media-uploads. 2)There is no limit on the number of userboxes, only the number of images uploaded for use in the userboxes. 3)Yes, every single image already on the site will need updating, though we'll cross that bridge when we come to it.
