Originally, the word ''Medjay'' used to refer to the inhabitants of the land of Medja, an ancient Nubian tribe located in the modern Sudan, which was subjugated and conscripted by the rulers of Egypt. The term later evolved to describe the Pharaoh's police forces during the New Kingdom period as Egyptians were integrated into the Nubian army, which was reformed into a more guardianship role.

By the time of the reign of Cleopatra, the role had faded away, with Bayek being the last Medjay and acting as a sheriff of sorts for his community.
