Well if you look at recent image uploads by Vatsa or Sima you should see what kind of info is needed and putting the information in is done by going into the New photo's section, selectong the image and pressing edit. then simply add the relevant information in either the template or in source mode for it. I am sorry if that isn't very clear (I'm not good at explaining things) but if you are still unsure user: Vatsa1708 is great at explaining it.
