That's the fanon Template.
The code for it is:
{{Fanon|Username|type=|extra=}}
Replace "Username" with yours and for type, you put "FanCanon" or "FanNon-Canon" depending on what it classifies as. For the extra part, you should add "Character", "Story" or "Animation" (without the quote marks).
