The Doctor (real name: Malfatto) is a multiplayer character in Assassin's Creed: Brotherhood, who uses a syringe as his primary weapon.
His outfit highly resembles the ones worn by doctors during the plague, and appears black in the dark. His face is concealed by a mask, therefore making his true identity unknown. His outfit is different from what the doctors wear in Assassin's Creed II, most noticeably due to the shorter beak of his mask.
He makes an appearance in the single player mode of Assassin's Creed: Brotherhood in a Templar Agent side mission, along with several of the other multiplayer characters.
He is also featured in Assassin's Creed: Project Legacy, where it is revealed that he is in the employ of Cesare Borgia, and is known as a serial killer that targets courtesans. Despite being on his side at the time, Fiora Cavazza feared him and called him a "monster." After their first meeting wherein Fiora delivered a sealed document from Cesare, Malfatto entered Fiora's home while she was sleeping and injected her with his syringe, either drugging her or attempting to poison her. However, she was able to defend herself, cutting him badly with her own weapon. Whether his attack was ordered by Cesare in the letter, or by Malfatto's own will is unknown.
At one point in Assassin's Creed: Brotherhood, Ezio Auditore comes across a courtesan crying over a fellow courtesan's dead body. She told him Malfatto had murdered her. Ezio reassured the courtesan that he would find Malfatto, and after a short time he located the doctor prowling in the district. Ezio signaled his Assassin Recruits to kill Malfatto, and they succeeded shortly after.

The only data I could find outside of that was a report of a masked doctor in 1502 seen lurking outside the Rosa in Fiore. This guy seems like really bad news, and I've read a lot of bad news lately.
Unique Moves
Lethal Injection
The Doctor charges at his target from behind and swiftly thrusts his syringe into the target's shoulder, injecting poison into their system. Before the victim can realize what has happened, they collapse to the ground and die silently. The Doctor can also perform a similar move from the front of the victim, where he thrusts the syringe into the victim's neck.
The Eye Stab
The Doctor charges and jumps onto his target, injecting the syringe into their eye and causing death.
