Yes it's just an injection, but it's unique towards the doctor and a silent kill (every character's silent kill animation is different, and this is one of the Doctor's) :D

The Docter pushes his target down onto his/her knees, and stabs him/her in the neck, possibly injecting him/her with poison straight into his/hers heart.
His outfit highly resembles the ones worn by doctors during the plague and appears black in the dark. His face is concealed by a mask, therefore making his true identity unknown. Strangely, his outfit appears to be different than what the doctors wear in Assassin's Creed II. The beak section of his mask is also shorter than the ones worn by the doctors in Assassin's Creed II. Ubisoft said that he will make an appearance in the single player mode of Assassin's Creed: Brotherhood along with the other multiplayer characters.
