Being a PC Gamer myself, I took the liberty of looking into your problem if you don't mind. Apparently, version 1.0 of the game has an issue with the launcher that causes the crash. See, when you switch to Desmond's Journey, ACRSP.exe (singleplayer) closes and ACRPR.exe (Penrose, code for the Animus Island missions) is supposed to be launched. The launcher issue causes ACRPR.exe to not launch properly, causing you to see it as a crash. Updating your game to version 1.01 fixes this issue. Also, launch the game using AssassinsCreedRevelations.exe and not ACRSP.exe . Hope I've helped and let me know if you need any help.
