I found it, a lot of places have the WRONG location in "The Sixth Day".

When you get into the final room with the big cross, drop down, and head forward. You'll see a chest on the right side, behind a fence... that is NOT the correct chest (I saw one picture, think it's here, that claims you get it after solving this room. THAT IS WRONG- you'll go get the chest now). Now, turn to the right, and there will be a tunnel entrance on the right side wall. Enter there, loot the chest for the Shrunken Head.

I've read you need to complete the memory to get the item, so just finish up now.
